Fibroblasts are important sentinel cells in the immune system and, here, it is proposed that
these cells play a critical role in the switch from acute inflammation to adaptive immunity and
tissue repair. It is suggested that chronic inflammation occurs because of disordered
fibroblast behaviour in which failure to switch off their inflammatory programme leads to the
inappropriate survival and retention of leukocytes within inflamed tissue.
